For nearly three decades, medical device manufacturers operating in the United States have looked to the Quality System Regulation (QSR)—codified under 21 CFR Part 820 —as their foundational blueprint for Current Good Manufacturing Practices (CGMP). 3. Combination Products Clarity 

The final rule provides clear cross-references to 21 CFR Part 4, explicitly defining how the QMSR elements apply to drug-device or biologic-device combination products, ensuring streamline alignment across regulatory thresholds. 

Mind the Gaps: Residual FDA-Specific Requirements 

While the FDA estimates that ISO 13485 covers roughly 80% of the previous QSR framework, compliance with ISO 13485:2016 alone does not automatically guarantee QMSR compliance. The FDA has retained several specific clauses to maintain its statutory oversight, creating “residual gaps” that manufacturers must meticulously address.

  • The Burden on SMEs: Small and medium-sized enterprises (SMEs) that previously operated exclusively within the U.S. market—and thus lack an established ISO 13485 system—face a steep, complex upward learning curve. 
  • The Problem of “Static” Integration: Because the FDA incorporated the 2016 version of ISO 13485 by reference, any future updates to the international standard will require an entirely new FDA rulemaking process. This introduces a risk of future divergence.
